Starting direct draw from scratch

I need some help in starting direct draw from scratch in Visual C++ 6? Ive got the Direct X 8 SDK files and want to do my own coding without using alot of the SDK source code in my program. What kind of projects do I need to start, MFC Appwizard or the C++ Source File? And how would I go about including any of header files like ddraw.h. Ive tried to finding out the projects files Direct X 8 SDK uses to build their direct draw applications but I am getting nowhere.
