USER RIGHTS TO FORMS,ASP.NET

Hi,
I am need of urgent code for ASP.NET,for the below requirement

a)There is a form with all form names listed with checkboxes beside it.

Refer the design shown below

FormName Read Write Read/Write

Dashboard Checkbox Checkbox Checkbox


Add project Checkbox Checkbox Checkbox


SAVE RESET



b)When we check the checkboxes,only those forms will be listed in menus.

How to do it?

I need the asp.net page design,code and DB structure.
