How to make a form with fields as per users choice

Hi,

I want to make my customer support page more focused to the user who is visiting it. For this I have planned the following:

The first field contains

I am 1- A visiting customer and I want to ask some question
2- A visiting customer and I want to add some comments
3- An active customer and I want to give additional info regarding my order
4- An active customer and I want to I want to ask some question about my order

For option 1 and 2 the following fields should appear:

1. Full Name
2. Email Address
3. Comments/Question

For option 3 and 4 the follwoing fields should appear

1. Order Number
2. Comments/Question

I want to ask if it is possible? If Yes, then how? All the fields are to be stored in a database aswell after submission. Any comments on the DB table design for this specific purpose are also welcome.

    use the onchange event handler in the select box to call a function which hides the non-needed fields
